General Concepts of LOSA

 

     A Line Operations Safety Audit (LOSA) is a formal process that requires expert and highly trained observers to observe operations in real-time and to collect safety-related data on environmental conditions, operational complexity, and human performance.

LOSA incorporates the principles of Threat and Error Management (TEM). TEM enables organizations to manage threats and errors so that those threats and errors do not turn into accidents and incidents in the future

LOSA is complementary to other reactive/proactive/predictive safety data collection programs such as Flight Operations Quality Assurance (FOQA; also known as Flight Data Monitoring [FDM]), Maintenance Operations Quality Assurance (MOQA), Maintenance Error Decision Aid (MEDA), Ramp Error Decision Aid (REDA), and Aviation Safety Action Program (ASAP)

LOSA Solutions

 

     Our robust LOSA Courses are designed to enable operators to implement a LOSA on their own. However, many operators choose to receive "a la carte" assistance from TACG. These a la carte solutions can be extremely beneficial in order to save time and resources and assure that your final product will be of the highest quality possible.

 

     Keep in mind that data collection is just part of the overall LOSA process. These data need to be cleaned/verified, analyzed, and written up in a professional report that will be read by management and other decision makers in your organization. TACG's a la carte LOSA solutions can target just one of these areas or we can do it all for you! Here is a list of our LOSA solutions:       

 

 

LOSA Software: Through our partner SMS Pro, we offer you your own dedicated, secure, LOSA database with a unique URL. LOSA observers can enter their observation forms directly into the database---either online or offline--- with a laptop, iPad, or even iPhone! (available for Flight Operation LOSA's only at this time. Other software options are available for Maintenance and Ramp LOSA's).

 

Line Operations Safety Audit Data Exchange (LOSADE): LOSADE is a benchmarking database that can be used to see how you are doing compared to other, similar operators. Operators that implement our LOSA software are automatically included in the LOSADE database. (available for Flight Operation LOSA's only at this time).

 

Collaborative Data Cleaning/Integrity Check: This is to assist you in verifying that all the data collected are valid (or "telling the truth"). This is an important step before the data analysis can be conducted.

 

LOSA Report Writing: We can write your full report and make it "ready to present to management." We have two report options:

 

Option 1) Basic data analysis and report writing which includes a summary of findings with basic (descriptive) statistical procedures.

 

Option 2) In-depth data analysis and report writing which includes a robust data drill-down, advanced (inferential) statistical tests, and comprehensive report.

 

Presentation of Findings: If we write your report (see above) we will deliver and present the final report to your Steering Committee upon completion of project.
