Discovery and scope. We clarify the event, objectives, stakeholders, available evidence, legal or regulatory considerations, timeline, and deliverables.

Evidence review. TACG examines relevant documentation, data, records, reports, procedures, and other available material.

Interviews and operational context. When included in the scope, we conduct structured discussions with personnel and evaluate how work is actually performed.

Integrated analysis. We assess active failures and latent conditions across technical, human, organizational, and SMS dimensions.

Findings and recommendations. We distinguish evidence from assumptions, identify contributing factors, and develop prioritized, practical actions.
